McKesson has provided care management services to Texas Medicaid clients for the past six years, generating a total of $40.1 million in net savings. The Personal Care Manager finds Medicaid clients at risk of developing chronic disease and identifies them as being high cost. Nurse managers can teach clients better ways to manage their conditions through personalized care plans and direct contact.
The company also offers the Diabetes Self-Management Training Program as a component of the Personal Care Manager for Medicaid patients.
