The “Achieving Patient-Centered Care and Optimized Health in Care Transitions by Evaluating the Value of Evidence” project, or Project ACHIEVE, uses a variety of qualitative and quantitative methods — including site visits, surveys and clinical and claims data from caregivers and providers — to identify the most important transitional care services and outcomes for patients. For its “Hospital Transitional Care Effort Adoption” survey, the partnership is seeking clinicians or staff in administration, case management and quality improvement from 300 hospitals who are willing to complete the survey and broaden the dataset.

The survey takes 15-20 minutes to complete can be found here.
