Pennsylvania health system collaborates with healthcare trust to improve care

The Dorothy Rider Pool Health Care Trust and Allentown, Pa.-based Lehigh Valley Health Network are collaborating to fund new clinical and community transformative programs that address social determinants of health or community-based patient care barriers, WFMZ-TV reported.

The Pool Trust has awarded $750,000 for these programs to fund projects ranging from $5,000 to $25,000.

The trust’s clinical transformational excellence award will give financial support to proposed projects that demonstrate potential to transform healthcare delivery and provide greater value to patients.

The award also will support projects that address social determinants of health, including challenges related to housing, mental health, food insecurity and education.  

“We are excited to work with The Dorothy Rider Pool Health Care Trust on innovative projects that have the potential to transform care delivery and improve the long-term health of our community,” said Debbie Salas-Lopez, MD, Lehigh Valley Health Network’s chief transformation officer.
