Patient Safety Tool: Pocket Card for Inpatient VTE Prophylaxis

A free venous thromboembolism prophylaxis pocket card is available from IPRO, the Medicare Quality Improvement Organization for New York state, to guide providers in choosing prophylactic measures for different surgeries.

The pocket card material was prepared by Stratis Health and the Oklahoma Foundation for Medical Quality, the Quality Improvement Organization Support Center for Patient Safety, and adapted by IPRO under contract with CMS.
