The Institute for Healthcare Improvement offers free resources to reduce the risk of several types of adverse events, including those related to medication errors.

The IHI Trigger Tool for Measuring Adverse Drug Events provides guidance for retrospective review of patient records to identify potential triggers that may lead to ADEs. The tool includes a list of known triggers as well as instructions for measuring the quantity and quality of ADEs.
