Patient engagement platform helps Baystate Health improve cardiac surgical outcomes

Springfield, Mass.-based Baystate Health has seen significant improvements in surgical outcomes since implementing SeamlessMD’s Optimal Care platform for cardiac patients.

The Optimal Care platform offers tools for patient engagement, care management and quality improvement that patients can access through their cellphones during the perioperative process. Baystate Health adopted the platform as part of its Cardiac Enhanced Recovery After Surgery program.

Baystate evaluated the outcomes of the first 60 patients to use the SeamlessMD app compared to a matched control group. They found length of stay fell 1.1 days, 30-day readmission rates dropped 22 percent, and discharges to skilled nursing facilities decreased 25 percent among patients using the app.

“This technology has clearly improved our ability to educate our patients both before and after their procedures,” Cheryl Crisafi, MSN, RN, a nurse coordinator of cardiac surgery for the Baystate Health Heart & Vascular Program. “The enthusiasm surrounding this platform has translated into rapid implementation of our ERAS pathways.”
