OIG investigation finds quality, safety weaknesses at Vermont VA hospital

During an investigation in December 2016, Department of Veterans Affairs Office of Inspector General found “certain system weaknesses” with quality and safety programs at White River (Vt.) Junction VA Medical Center, among other issues, according to a report released Tuesday.

The inspectors found problems with the hospital’s sedation and anticoagulation care and management of disruptive or violent behavior, as well as with the oversight and decision-making of the hospital’s quality management board, among other issues.

In a statement given to New Hampshire Public Radio, the hospital has “made substantial progress towards closing out the items identified by OIG as opportunities for improvement.”
