Nexus Health Resources, Orange Regional Medical Center develop pulmonary disease program

Nexus Health Resources has announced it will expand its transitional care services for patients at Orange Regional Medical Center in Middletown, N.Y.

Nexus Health provides critical post-hospitalization services to congestive heart failure and pneumonia patients from ORMC to avoid readmissions and boost recovery. Under the expanded contract, Nexus Health’s services will also be offered to patients with chronic obstructive pulmonary disease.

ORMC’s Vice President of Patient Safety and Quality & Health Information Management Rose Baczewski spoke very highly of Nexus Health’s services.

“Nexus Health has provided ORMC and our patients with dynamic results — along with high-quality transitional care management and coordination,’ said Ms. Baczewski. “This success helped to shape ORMC’s decision in expanding Nexus Health’s scope of work to discharged patients with COPD.”
