New York hospital receives Magnet prize

The American Nurses Credentialing Center has awarded White Plains (N.Y.) Hospital the 2025 ANCC Magnet Prize for its innovation in care delivery, according to an Oct. 8 news release. 

The prize, sponsored by Press Ganey, recognizes a Magnet-designated organization’s nursing team for exemplary innovation in patient care delivery that improves outcomes. 

White Plains Hospital was honored for a nurse-led project that uses remote patient monitoring to support patients with poorly managed diabetes after discharge. The 90-day transitional care model incorporates continuous glucose monitoring, clinical coaching and behavioral support, the release said. 

The model also addresses food insecurity, transportation and medication access.

With the $125,000 award, White Plains Hospital will expand access, launch research and create a toolkit to standardize the model.
