National Academy of Medicine to recognize 15th anniversary of ‘To Err is Human’

The National Academy of Medicine will recognize the 15th anniversary of the landmark Institute of Medicine report, “To Err is Human: Building a Safer Health System,” at its 2015 Richard & Hinda Rosenthal Symposium.

The 15th anniversary of the Institute of Medicine report, “Crossing the Quality Chasm: A New Health System for the 21st Century,” will also be recognized during the full-day event.

During the symposium — which is scheduled for Dec. 10 — healthcare professionals will examine the recent gains, remaining challenges and future directions in patient safety and care quality.
