More Than 1.5k Primary Care Physicians Enroll in BCBS Florida’s Medical Home

Blue Cross and Blue Shield of Florida has launched its statewide patient-centered medical home program, in which more than 1,500 primary care physicians have already enrolled.

Collectively, the program will serve roughly 530,000 patients covered by BCBS. Initially, the PCMH program will target chronic conditions such as diabetes, chronic obstructive pulmonary disease, coronary artery disease, asthma and congestive heart failure.

Enrolled physicians will receive additional compensation from BCBS Florida based on overall patient results and their adherence to PCMH guidelines.
