The PCMH initiative builds upon HANYS’ success with the National Committee for Quality Assurance Diabetes Recognition Program. Under the latter program, HANYS has helped more than 750 providers achieve PCMH recognition.
By collaborating with a healthcare assessment group, HANYS will provide consulting support, tools and assessment for hospital-based clinics and primary care sites pursuing PCMH recognition. Clinics and sites that receive the recognition will be eligible for enhanced fee-for-service add-on Medicaid incentive payments.
