Geisinger Health System developed a patient-centered medical home model called ProvenHealth Navigators. Forty-three primary care clinics were converted into this model from 2006 to 2010. Researchers found this model was associated with lower total healthcare costs and sustained cost savings over time. Specifically, the model led to a total cost savings of 7.1 percent when accounting for effects from prescription drug coverage.
