The program, which began in December, is focused on enhanced care coordination through collaboration with provider organizations in California. It aims to improve the health of patients with chronic conditions and improve the value of care delivered by the providers.
Anthem will partner with care providers to form personal health teams for members with chronic conditions like diabetes, asthma and high blood pressure. The care team will coordinate each member’s care based on his or her needs.
The program will eventually be available at more than 20 provider locations, and is currently available through Cedars-Sinai Medical Care Foundation in Beverly Hills, Calif., and at Torrance (Calif.) Memorial Medical Center.
