The Agency for Healthcare Research and Quality has released an infographic on health literacy pathways inspired by a February 2013 Health Affairs blog post, “A Proposed ‘Health Literate Care Model’ Would Constitute A Systems Approach to Improving Patients’ Engagement in Care.”
