8 actions for pharmacists to limit medication-related adverse events

On Tuesday, the American Pharmacists Association made a new resource available to assist pharmacists in ensuring optimal medication-related outcomes for patients.

The new APhA resource — Pharmacists’ Impact on Patient Safety — focuses on eight actions pharmacists can take to improve patient experience and limit adverse events related to medications.

The eight actions are:

1. Ensuring affordable access to medications

2. Providing sufficient medication information to patients and caregivers

3. Evaluating medication appropriateness

4. Helping patients adhere to medication prescriptions

5. Delivering care services to improve patient health and wellness

6. Performing comprehensive reviews of all a patient’s medication

7. Assessing the patient’s health and determining the likely effectiveness of medication therapy

8. Assisting with medication management as patients undergo care transitions

Chelsea M. Anderson, PharmD, chair of the panel that prepared the report, said, “The most important thing about patient safety overall is that it requires a team effort. Pharmacists are definitely a part of that team, and we should let others know how pharmacists help improve the quality and safety of patient care.”
