The six communities — Santa Cruz County, Calif.; New Orleans; Cambridge, Mass.; Fall River, Mass.; Manistique, Mich.; and Minneapolis — were chosen from among more than 160 applicants for creating partnerships and developing innovative strategies to improve community health. Each community received $25,000 to continue their work on community health.
The prize is part of the County Health Rankings & Roadmaps program, which educates the public and policymakers on community health issues.
