The following is a list of hospitals with the lowest 30-day readmission rates from pneumonia, according to data from CMS.

The information was pulled from CMS' Hospital Compare website and is from July 2012 through June 2015, the most recent data available.

All hospitals on this list have a pneumonia 30-day readmission rate of less than 14.2 percent. For reference, the national average rate is 17.1 percent.
