New York City-based Maimonides Health cut unnecessary readmissions and hospital bed days with an acute care at home program.
Maimonides Health launched a partnership with myLaurel, a home-based acute and transitional care company, in May 2025 to improve care delivery for frail, medically complex and older patients, according to a Sept. 16 system news release.
In one year, the program served more than 1,200 patients in their homes and provided in-home clinical interventions, virtual care, diagnostics and therapeutics. Patients in the program had a 43% reduction in 30-day readmissions compared to the standard medicine service line — which equates to more than 1,000 unnecessary readmissions avoided and 2.29 hospital bed days saved per patient.
The system plans to expand the program to additional care sites and specialty service lines.
