The Institute for Healthcare Improvement named 14 systems in the U.S. to achieve Age-Friendly Health Systems Systemwide Spread.
From 2024 to 2025, IHI convened 30 systems in the Age-Friendly System-wide Spread Collaborative. The collaborative focused on embedding the 4Ms model — what matters, medications, mentation and mobility — systemwide to have an equitable impact on older adults across multiple sites and care settings, according to an association news release.
Although 16 of the participating systems did not reach the level of systemwide spread, IHI recognized them for making strides in age-friendly care and sharing deep learnings in the collaborative.
