The Aetna Medicare Northern New Jersey Prime Plan will offer access to high-quality, affordable and coordinated care through Atlantic Health System in Morristown, Hackensack University Health Network and Hunterson Healthcare in Flemington, as well as their affiliated providers.
The new plan’s model involves nurse case managers collaborating with the beneficiary’s primary care physician to assist in complex disease management and care.
The Aetna plan will be available in Bergen, Essex, Hudson, Hunterdon, Morris, Passaic, Somerset, Sussex, Union and Warren counties during the open enrollment period this fall for January 2015 effective dates.
