CMS extended the reporting period from its original Dec. 14 deadline to give hospitals more time to voluntarily submit data on discharges that occurred between Jan. 1, 2018, and June 30, 2018.
Unlike CMS’ 30-day readmission measure, the hybrid measure uses both EHR data and claims data to calculate a hospital’s risk-standardized readmission rate.
Before submitting data to CMS, hospitals can use QualityNet’s Pre-Submission Validation Application tool to ensure proper formatting.
