In the Aug. 12 complaint, the patient also accused the U.S. Pain & Spine Institute, USPS Healthcare, USPS Surgical Institute, PICIG Management, Waterway Medical Management and Phillip Phan, MD, of breach of contract.
The patient says she had a spinal cord device implanted Oct. 10, 2017 at the spine hospital. She has been requesting copies of her medical and imaging records but claims the hospital has refused to comply.
U.S. Pain & Spine Hospital say that they do not have the information on hand that the patient is requesting.
The patient is seeking attorney fees, court costs, all equitable relief and interest.
