Former Crozer Employee Pleads Guilty to Stealing Patient Identities

Reynaldo Estrada, a former employee of Crozer-Chester (Pa.) MedicalCenter and Chester, Pa.-based Community Hospital, has pleaded guilty to stealing hospital patients’ identities as part of a tax fraud scheme, according to a Philadelphia Business Journal report.

From October 2010 to October 2011, Mr. Estrada stole “scores” of treatment authorization forms from both Crozer-Chester Medical Center and Community Hospital. The forms contained patients’ names, addresses dates of birth and Social Security numbers, according to the report.

Mr. Estrada was paid by Rafael Henriquez Polanco and Yanira Lopez, co-conspirators in the tax fraud scheme, for the stolen forms. The co-conspirators claimed more than $1.7 million in income tax refunds from the approximately 144 fraudulent and false income tax returns they filed using the patient information Mr. Estrada had stolen, according to the report.
