Dr. Lee Adam Balaklaw Charged With Medicaid Fraud

A Lawrence County grand jury has indicted anesthesiologist Lee Adam Balaklaw, MD, of allegedly intentionally or wantonly submitting fraudulent or false claims to Medicaid totaling $90,000 for 29 patients, according to a WSAZ report.
