The goals of the program are to improve care transitions, increase quality of care, reduce readmissions and demonstrate savings to the Medicare program. In January, CMS added 35 participant sites to the 17 that were announced in August. The sites include the San Diego Care Transitions Partnership, which consists of a partnership between community organizations and four health systems: Scripps Health and Sharp Healthcare, both based in San Diego, Escondido-based Palomar Health and the University of California San Diego Health System.
AccessCare, a program based in Morrisville, N.C., was also selected and includes hospitals from Chapel Hill-based UNC Health Care, Durham-based Duke Health and Raleigh-based WakeMed Health & Hospitals.
