Advocate Aurora Health, Oak Street Health partner on primary clinic for Medicare patients

Milwaukee-based Advocate Aurora Health is partnering with Oak Street Health to open a primary care clinic in Elgin, Ill., that will serve Medicare enrollees.

The clinic will offer comprehensive primary care services, flexible scheduling, more face time with physicians, transportation to office visits, insurance education, community wellness events and a care coordination team.

“By shifting focus toward quality of care instead of volume of services, there has been a meaningful improvement in reducing hospital admissions and emergency room visits,” said Mike Pykosz, CEO and co-founder of Oak Street Health. “We know Advocate Aurora Health shares that vision, and as a national leader in population health, is the right partner to expand our services in the northwest suburbs of Chicago.”

The clinic is set to open in April.
