LifeBridge to launch practice focused on transitions of care

Baltimore-based LifeBridge Health plans to introduce a new practice called Post-Acute Physician Partners, with the aim of improving care transitions after patients leave the hospital, according to The Baltimore Sun.

Post-Acute Physician Partners will establish a multi-state network of primary care physicians, specialists, nurse practitioners and physician assistants who will work in existing facilities. The teams will focus full-time on decreasing complications, overseeing medication management and smoothing transitions after the patient leaves the hospital for a rehabilitation home, nursing home or their own home, according to the report.
