Researchers at North Shore University Hospital in Manhasset, N.Y., designed and implemented the “Follow Your Heart” program, a transitional care program providing continuity of care, a communication hub and medication management to discharged patients who have undergone coronary artery bypass graft operations.
Patients enrolled in the “Follow Your Heart” program experienced a 3.85 percent 30-day readmission or death rate, compared to 11.54 percent with patients receiving usual care.
Researchers concluded a home transition program significantly reduced the 30-day readmission/death rate. They suggest reallocating resources may help further improve readmission and mortality rates as well as be applicable to other patient groups.
