Oswego (N.Y.) Health CEO Chuck Gijanto is leaving his post, according to an Oswego County Today report.

Here are four things to know:

1. Mr. Gijanto has served as CEO for a little over one year, beginning in September 2015.

2. The hospital’s board of directors offered to extend Mr. Gijanto’s contract, but he could not accept “due to personal reasons,” according to the report.

3. Mr. Gijanto will continue to serve at the hospital’s helm until the hospital selects replacement.

4. The hospital’s board has established a search committee and engaged an external search firm. Mr. Gijanto will assist the search committee.
