A year after the partnership went live in January 2023, HealthSnap reported Jan. 11 that both the remote patient monitoring and chronic care management have been successfully delivered to more than 25,000 patients.
UnityPoint Health attributes better outcomes in patients living with a chronic illness to the ability to stay connected between visits with remote patient monitoring. In 2023, the health system only operated HealthSnap’s remote patient monitoring and chronic care management in the family medicine and internal medicine specialties, but will expand to more departments in the upcoming year.
