Truman Medical Centers taps Cerner for chronic disease management

Kansas City, Mo.-based Truman Medical Centers on Tuesday unveiled a pilot program to improve the health of patients with high-risk chronic conditions.

The pilot program leverages algorithms from HealtheIntent — Cerner’s population health management platform — to identify patients who might benefit from remote patient monitoring, such as those with diabetes or heart failure.

These patients will receive remote monitoring devices that connect to Truman Medical Centers’ EHR system. HealtheIntent will analyze this health information, which care teams can use to proactively intervene in patient treatment.

In six to nine months, Truman Medical Centers will assess whether patients’ health metrics improved during the pilot program.
