The report, titled Strengthening Chronic Care, was compiled from two recent surveys on chronic disease management. The first, conducted by Kelton Research on behalf of West, surveyed 502 adults with at least one chronic health condition and who had been hospitalized as a result of a chronic illness. The second, administered by West, surveyed 417 healthcare providers.
Here are five of the report’s findings.
- Thirty-nine percent of patients report that they are only somewhat knowledgeable about how to manage their condition.
- The majority (70 percent) of patients are interested in receiving more resources to help manage their condition.
- Just over half (54 percent) of patients think checking in with their provider once or twice a week would be valuable. One in five patients report wanting 24-hour assistance managing their condition.
- Patients are also interested in at-home monitoring devices, with 50 percent reporting one-way devices are useful and 58 percent reporting two-way devices are useful.
- When asked to choose between one-way or two-way monitoring devices, slightly more than half (53 percent) of patients preferred two-way devices.
