Here are three things to know about the partnership.

1. The Care Intelligence System standardizes data from across a healthcare organization to bring providers actionable insights. For example, it supports organizations in applying predictive and prescriptive analytics to help providers better understand at-risk patients populations and performance metrics.

2. StratiFi Health partners with physicians to shape patients’ care paths as they navigate the healthcare system. The company’s population health division now leverages the Care Intelligence System to identify at-risk populations and streamline care coordination accordingly.

3. The platform provides care management teams with standardized medical information for all patients, including performance metrics based on 20 quality measures. It also supports StratiFi Health as it negotiates payer contracts with information about performance monitoring.
