Omada Health aims to reduce chronic disease in employee and health plan member populations. It will offer the devices to help individuals who have been clinically recommended to self-monitor blood pressure and glucose levels. With their cellular devices, individuals can upload their readings in real-time so their Omada coaches can better personalize their care plans. Omada coaches are individuals with experience in areas including diet and fitness that help Omada program participants manage their chronic diseases.
“Participants in the Omada program build deep relationships with their coaches; enabling automatic cellular uploading of glucose and blood pressure data will enable those coaches to provide guidance informed by a wide range of data and insights — helping support the best possible health outcomes,” Omada Vice President of Medical Affairs Carolyn Bradner Jasik, MD, said in a news release.
