The organization discovered the potential breach through a routine internal compliance audit Feb. 25. It found an employee had emailed a report of recent referrals and admission activity to himself or herself through an unsecured channel in December 2012 and February 2013.
The report included patient names, referral sources, names of insurance companies, admission dates and dates of discharge. It did not include Social Security numbers, dates of birth or addresses.
The organization said the entire staff has received additional training in response to this incident, and the organization is performing a comprehensive review of its policies.
