According to the report, documents were to have been shredded before being moved from a building scheduled for demolition. However, an individual saw papers containing cashier’s receipts for credit card payments made by five patients at St. Elizabeth’s outpatient surgical center flying through a field and reported the incident to the hospital. The receipts included patients’ names, hospital account numbers and credit card information.
St. Elizabeth’s has no evidence that information for more than five patients was lost. However, the hospital has notified all patients whose information was stored in the office where the five patients’ records were kept.
