Visionworks is a retail eyeglasses store chain.
The server was replaced for scheduled upgrades on June 2, and, according to a Penn Live report, was discovered missing when it was recalled to have the data erased. Visionworks believes the server was sent to one of the store’s landfills, and says there is no reason to believe any of the stored information has been inappropriately used or accessed.
The server contained patient names, addresses and some health-related information regarding optometrist visits and lens prescriptions, according to the report. Additionally, the server contained credit card information, though it is encrypted, so Visionworks does not believe that information to be at risk.
