Granger Medical Clinic Notifies 2,600 Patients, Federal Officials of Possible Data Breach

West Valley City, Utah-based Granger Medical Clinic has notified federal health officials of a possible data breach after 2,600 medical appointment records went missing, according to a report by The Salt Lake Tribune.

The records were meant to be shredded and were discovered to be missing on Jan. 22. All records were from 2012 and included names of patients, dates of appointments and reasons for medical visit. No addresses or Social Security numbers were included in the records.

Letters have been sent to the affected patients. To date there has been no indication that any of the information has been misused, according to the report.
