Mercy, based in Chesterfield, Mo., has entered a 20-year affiliation with Wellvana to broaden access to value-based care programs for independent primary care physicians and advanced practice providers across its service areas.
Here are four things to know about the affiliation:
- The partnership combines Mercy’s population health infrastructure with Wellvana’s high-touch enablement platform, creating what the organizations expect could become one of the nation’s largest and top-performing clinically integrated networks, according to a Dec. 4 news release.
- The effort targets clinicians who are not employed by Mercy but want access to coordinated care tools and alternative payment models that align with high-quality care.
- Participating providers can receive support such as electronic medical record integration, care coordination teams, data-driven insights and operational assistance at the practice level. The program is designed to help providers manage patients covered by Medicare, Medicare Advantage and commercial insurance plans.
- Value-based care models reward providers for quality and outcomes rather than service volume. Mercy and Wellvana said expanding participation among independent providers could reduce administrative burden, improve performance and broaden patient access to coordinated, proactive care.
