Three details:
1. On Jan. 12, the practice reported an unauthorized individual gained access to an employee’s email account last year and attempted to divert funds. The attempt was unsuccessful and the practice disabled the affected email account.
2. The practice discovered on Nov. 20 that personal information of 20,787 patients was accessible in the hacked email account.
3. Patient names, addresses, birth dates and health information were accessible in the account, although the practice does not have evidence the information was misused.
