For the survey, the professional services firm Grant Thornton interviewed and surveyed 313 IT executives from 29 federal agencies, including CIOs, chief information security officers and chief data officers.

1. Vulnerabilities from aging applications and technologies
2. Human error
3. Malware
4. Phishing campaigns
5. Internet-facing attacks, such as distributed denial-of-service attacks
