In a media notice, Dr. Benjamin said the paper charts were stolen in November. After notifying the police, a suspect was identified and taken into custody, and some documents were recovered.
According to the notice, potentially compromised information includes patient names, addresses, phone numbers, birth dates, marital statuses, spouses’ names, Social Security numbers, referring physicians, insurance information, lab values, height, weight and blood pressure.
Dr. Benjamin’s practices are in West Hills, Calif., and Simi Valley, Calif. He reported the data breach to HHS Office of Civil Rights’ data breach portal Dec. 28.
