The mailing error occurred Dec. 9, and Borgess learned of the incident Dec. 10, according to the report.
Letters contain patient names and indicate that the patients have been treated at Borgess Rheumatology, but they do not contain Social Security numbers, according to Lew Tysman, a spokesman for Borgess.
