Edison, N.J.-based Hackensack Meridian Health is expanding its older adult care strategy through a new partnership with Cadence, a remote care technology company that uses artificial intelligence to monitor patients’ health outside the hospital.
The initiative aims to help older adults manage chronic conditions at home by using connected devices that track blood pressure, heart rate, weight and blood glucose levels. Cadence’s AI platform reviews patient data in real time and alerts a 24/7 clinical team when intervention may be needed, according to an Oct. 21 news release Hackensack Meridian shared with Becker’s.
The program, which is already monitoring more than 1,000 patients, functions as an extension of the health system’s clinical operations. Cadence’s team oversees daily monitoring and patient follow-up, surfacing only the most urgent cases to Hackensack Meridian Health providers.
