  1. Extend reach. Eleanor enables you to engage more people without increasing headcount. She’s already made more than two million calls (about a third of them in Spanish) with an average duration of almost 3.5 minutes. For example, one health system used Eleanor to improve annual wellness visit attendance. Within two weeks, she achieved the organization’s monthly goal for outreach, freeing staff time to address other activities.
  2. Ease the case-load burden. Having Eleanor initiate clinical assessments and engagement optimizes clinician time. New England Life Care had Eleanor contact patients starting total parenteral nutrition infusion therapy. Eighty-nine percent of the people she spoke to completed the pre-visit assessment with Eleanor only. Those needing support were connected to a care team member, so providers could focus on top-of-license work.
  3. Improve chronic care management. Monitoring patients between visits has always been a challenge. When a New York health system wanted to improve ongoing patient monitoring, it deployed Eleanor to identify changes in status and chart side effects and complications. She flagged 15% of the patients for intervention by a care manager. As a result, hospitalizations among this patient population decreased by 33%.
  4. Reduce costly no-shows. At Montefiore Einstein Comprehensive Cancer Center, Eleanor reached out to colonoscopy no-shows, nearly doubling completion rates from 10% to 19%, with an overall 36% increase in patient volume.
  5. Identify and address barriers to care: Montefiore enlisted Eleanor to uncover the factors causing no-shows. She shared patients’ insights on harmful SDOH with navigators who worked directly with people to improve access to appropriate care and support better health and quality of life. Because of this combined effort, 25% of patients transferred to a navigator got their screenings, almost doubling no-show completions.
  6. Decrease readmissions. Augmenting predischarge interventions with post-discharge follow-up phone calls can limit adverse events and lower unnecessary utilization. Eleanor called patients for a New York-area system and reached 75% in two days and enrolled 90% in a 30-day monitoring program — an effort that reduced 30-day readmissions by 22%.
