Nashville, Tenn.-based HCA Healthcare has eliminated a “small percentage” of positions across its corporate office and support functions, a spokesperson for the publicly traded for-profit system confirmed to Becker’s Sept. 2.
The spokesperson did not specify the exact number of positions affected. In a statement, the spokesperson said that “hospitals nationwide are navigating changes including shifts in healthcare policy, an increase in uninsured patients and rising costs.”
“We have a responsibility to continually evaluate how we operate and deploy resources so HCA Healthcare remains strong and positioned to serve our patients and communities for the long term,” the spokesperson said. “These targeted changes are part of that work.”
HCA revised its 2026 financial guidance downward in July. The 190-hospital system narrowed its revenue outlook to a range of $77 billion to $79.5 billion, from its January projection of $76.5 billion to $80 billion. It also lowered its net income outlook to between $6.3 billion and $6.7 billion, down from the prior range of $6.5 billion to $7 billion.
That revision came as HCA raised its estimate of the financial hit from ACA-related changes to $1 billion to $1.2 billion in 2026, up from an April estimate of $600 million to $900 million.
“As we look to the balance of the year, we have adjusted our guidance to reflect these factors,” HCA CEO Sam Hazen said in a July 14 news release. “Moreover, we remain confident in our ability to navigate through this dynamic environment, maintain our focus and investments on improving patient care, and execute on our strategic plan to digitize and grow our healthcare networks.”
The spokesperson told Becker’s the system will support affected employees through the transition. Those affected will be provided with resources including severance and recruitment and outplacement support as they pursue new employment opportunities.
“At the same time, we continue to invest in our people and patient care and are hiring for roles across the country to deliver on our mission and meet the needs of the communities we serve,” the spokesperson said.
HCA employs more than 320,000 people, according to its website. The system reported operating income of $2.5 billion (12.3% operating margin) in the second quarter of 2026, compared to $2.4 billion (13% margin) during the same period last year.
