Three things to know:
1. Under the new value-based relationship, UnitedHealthcare will provide SEHP data, such as members’ underlying medical conditions, past treatment, care gaps and medications, to inform treatment for approximately 16,000 patients.
2. The data-sharing will allow SEHP to reduce duplicate tests and improve physician coordination across its more than 11 hospitals and about 2,000 providers.
3. Additionally, the program aims to identify patients at high risk for chronic disease and help reduce unnecessary emergency room visits and hospital readmissions, the organizations said.
