Toronto health network shuts down electronic system after patient record mix-ups

Toronto-based University Health Network is in the midst of reviewing five years’ worth of online patient records after officials discovered some patients’ information had been misplaced, CTV News reports.

University Health Network officials began an internal investigation after identifying four cases in which one patient’s medical information was misfiled into a separate patient’s electronic record. Officials said they have notified the four patients whose information was misplaced.

University Health Network attributed the incidents to an “electronic glitch” in its computer network, causing the health network to temporarily shut down its electronic system, according to CTV News. Officials said the network intends to keep the system closed while they review logs of patient records.
