The CAC initiative will cover about 5,500 Cigna members who receive care from Valley Preferred primary care physicians and specialists.

As part of the initiative, Valley Preferred registered nurses will serve as clinical care coordinators who will help patients with chronic disease and other health challenges navigate the care system. The care coordinators will work with Cigna case managers to ensure collaboration between the two organizations.

Cigna will pay Valley Preferred physicians for medical care and care coordination services, and the physicians can also be rewarded on a pay-for-value structure if they meet quality and cost targets.

This is Cigna’s first CAC initiative in Pennsylvania. As of July 1, Cigna will have 66 CAC initiatives in 26 states.
